Thank you for considering Yakima Integrative Health! We are currently accepting new patients for our clinic. To be added to the waitlist to be registered as a patient, please fill out this form. If there are multiple patients to be registered, please fill out the form once for each patient.

Please be advised that we cannot bill Medicare or any Medicare supplemental plans. If your insurance is Medicare, or if it is not listed in those above, we cannot bill them and you will be responsible in full for payment. Our self-pay price discounts are as follows.

  • Regular Office Visit:
    • Attending: $155.00
    • Resident: $110.00
  • Annual Exam:
    • Attending: $190.00

  • Well Child Exam:

  • Acute:
    • Attending: $95.00

  • Acupuncture: $110.00
  • Massage Therapy (60 min): $85.00

Waitlist Form

This information will be used to register you into our system as a prospective patient. Please ensure that all information is correctly entered, especially the phone number. If your phone number is incorrectly entered, we will send an email to obtain the correct contact. This email might be filtered into spam, so if you haven’t heard from us within two weeks please contact our front office. Once we have you registered into our system, you will receive a call to let you know that you are registered and may call to schedule.

After Registration

Once you’ve received the call that you are registered into our system and you’ve been scheduled, the next step is to either register for the online portal and complete check-in or to fill out the new patient paperwork.
